See doctor ASAP: Appendicitis needs be ruled out for anyone with severe right lower quadrant pain especially when associated with a fever. Even though spotting is not part of the picture, you need to consult your doctor or get to the ER. Ectopic pregnancy is another possibility but either condition requires immediate medical attention.
