Unknown: You can get a long list of causes, but it is mostly speculation. This is important because specific treatment is seldom available. Aging and pregnancy are associated with higher incidence. Quinine and quinidine have been used in the past, but sometimes cause serious toxicity. Immediate walking and/or massage usually helps. Any other treatment is of questionable use. Causes are UNKNOWN.
