For an hcg test, how is it determined which category you are put into; pregnant, non pregnant, postmenopausal?

Have to check HCG: Ha, not trying to be funny but the HCG test is a pregnancy test so it would have to be done before you can know whether someone is pregnant or not unless, of course its obvious by the woman's appearance. In someone who is pregnant the level of HCG is used to determine if pregnancy is normal. After menopause it would only be useful to track an extremely rare tumor. If normal and not pregnant HCG=0
