Can a person physically feel it when hypertension medication starts to wear off after it well passes its medication half life?

Possibly: In most patients hypertension is silent, but when there is a rather sudden rise in blood pressure, headache or some type of dizziness may be noticeable. Also, if the medication wearing off results in a rise in heart rate, palpitations may be noticed.
