Middle ear infection: A common cause of ear pain at any age would be pressure on the ear drum from puss during a middle ear infection. That process would not be effected by drops and may never respond to acetaminophen. Proper assessment would require an exam. Sometimes a bad tooth can have an odd deferred pain that shows up as an earache.
