Two genes neeeded: It doesn't create the disorder but for many conditions for there to be symptoms a particular gene must appear twice. In incest the two parents share many genes and so are more likely to have overlapping genes which will be duplicated in the children. Hemophilia, which requires this, led to the deaths of many european monarchs who shared the gene.
