Stricture/stenosis: While there could be several explanations for this, urethral stricture or stenosis of the urethral meatus are the two most likely. Both involve scarring and narrowing of the urethra and lead to a turbulent and/or deflected urethral stream. There are several tests to help determine if this is the cause. Probably best to get it checked out.
