Chaffing?: Does this occur after exercising/running? sometimes nipples if erect can rub on the inside of the shirt and cause some chaffing. If you're not having discharge, you can try using vaseline on the nipples to help avoid this problem

Nipple problems: Males can get nipple eczema which is treatable. However some rarer conditions causing breast development (gynaecomastia) may need to be considered and investigated/excluded. Please see your doctor for examination as you may require further tests

Irritant?: Sometimes a new washing powder or fabric conditioner can be a trigger for skin irritation. Runners often get irritated nipples from the friction caused by fabric.
