Severe = emergency: Severe pain is an emergency, and one can consider calling 911 for an ambulance. Many things are in the abdomen: stomach, pancreas, liver, small intestine, gall bladder, large intestine, abdominal muscles, etc... A doctor's exam can help figure things out. Pain in the right upper abdomen can be due to gall bladder disease or gallstones. Pain in the right lower abdomen can be due to appendicitis.
