Dermatologist needed: Here’s the deal: if a problem is recurrent, or persistent, you need to see a dermatologist. You may need additional workup, including labs, but do see one ASAP, to see what medicines you may need.

It's neglected : This is the 21st century and acne is a serious personal and professional liability. Looking for causes and explanations for acne is futile. If you have nodular acne, it needs to be treated aggressively usually with isotretinoin. Anyone who tells you otherwise is wrong. Your face will be mutilated if this keeps up. Exercise basic self-care and get treated.
