Surgery last option: Before jumping to surgery, most cases similar to what you describe should have a serious trial of at least six weeks of physical therapy to build up the muscles and tendons of the rotator cuff. This may include therapeutic ultrasound also. The goals are pain relief, strengthening and improvement of range of motion. Bursal injection may help too. Improvement takes time.
