Nipple eczema?: Could be possible but also need to think about the more nasty possibility of the lump being a small breast abscess (although other less serious cysts are certainly possible). Needs a doctor examination to make the diagnosis and arrange treatment. This needs to be done urgently if fevers/unwell.

Breast exam/us: Your symptoms are serious and require a breast examination, bilateral breast ultrasound and probable mammogram. Call your gYN or pCP to set up an appointment and do not ignore these symptoms.
