Blisters from: genital herpes would hurt around the area as much as within the blister itself. this is the nature of genital herpes. Most of the lesions sit on top of a cutaneous skin nerve and therefore transmit pain in the area that it serves.

Either way: Either pattern of pain is possible. Herpes infrequently appears as single lesions, especially with the first infection. It can happen, but unlikely.
