Not much!: Damage with holes in the lungs never heal much. THe holes stay. The lungs ability to regenerate is minimal. However, there are several points that depend on the particulars: 1) if it is TB, the disease needs to be treated. 2) if due to damage, like cigarettes, the cause of damage needs to be removed. 3) if the holes are large, there is some risk of stuff getting in and growing in them.
