Exam needed: yes, you need to have an ear exam done soon. please see a provider whether your PCP or at the urgent care.

Perforated ear drum: If increasing pain follow up with ENT as there may be infection. use cotton ball with petroleum jelly or ear plugs to prevent water from entering the ear., Avoid swimming /diving.
