Unlikely: The cited failure rate of the pill is about 5% per year of use. If you are consistent with timing & never forget, it is likely lower for you. I would encourage BF to go shopping for some sturdier condoms with a better fit. Doubling up is a good idea for all concerned.

Ortho cyclen: Ortho cyclen birth control pills are 99% effective in preventing pregnancy. The main reason to use condoms while on the pill is to prevent STD/STI transmission. If you are taking the pill regularly and not missing any days it is highly unlikely that you can get pregnant.
