Unpredictable: It doesn't make any difference whether you try to stimulate the reflex in a normotensive vs a hypertensive individual. The reflex is engaged by pulling on an extraocular muscle during surgery. You are not likely to engage it by pushing on the eye. Should you stimulate the reflex, the extreme slowdown in heart rate can be prolonged and lead to ischemia of the brain, kidney and heart.
