Monitored: Your child is young enough that her excursions to the bathroom should be monitored so you are aware of her health. She's learning, so may not know how to interpret her body signals.

Bladder & bowel : incontinence till age 6 can be a medical, developmental or behavioral problem. Possibilities include malformations of the spine & sacrum that impact the nerves that send signals of the need to urinate or defecate & those that function to empty the bladder & lower bowel. Her pediatrician can determine if she needs X-rays or referrals to specialists.
