Orgasm: if no persistent pain or pain during sexual intercourse probably just muscle contraction. I wouldn't worry at the moment.

Orgasm contractions: Organism can cause rapid and intense smooth muscle contractions of the uterus and vagina. If there ae adhesions from a previous surgery then pulling or tugging may also be present when orgasm occurs. If that is not the case, then chalk it up to a new "level of orgiastic pleasure".
