Possible: simethicone and gripe water are basically meant for adults or older adolescents for bloating and flatulence. Anyone can have an allergic reaction to anything. Your child's symptoms could be such an allergic reaction if he/she has been exposed to those agents. if the condition persists, your PCP needs to evaluate.

Black/grayish spots: Black/grayish spots on tongue are not an allergic reaction to simethicone or gripe water. Spots are due to chemical reaction of agents on the tongue. Both are used extensively in infants.
