The emergency room can evaluate severe stomach pain, if the primary care or regular doctor is not able to help right away. Possible causes include appendicitis, stomach ulcer, gallstones, kidney stones, pancreatitis, twisted bowel, perforated bowel, mesenteric adenitis, twisted ovary, pregnancy-related problems, etc...
