Swollen lymph nodes under both jaw. dentist found infection on x-ray. 7 days out of 10 on penicillin w/ no change. at what point do i need a biopsy?

Swollen nodes: Your swollen nodes may or may not be related to your dental infection. There is no specific time table regarding when to biopsy a suspected node which is indicated for a growth, not an infection. It is up to your own dentist to determine the proper course of action. Was cause of infection treated? Antibiotics alone cannot solve problem. Sometimes change in antibiotic or dose is also necessary.

Oral Surgeon : You need to see an Oral and Maxillofacial Surgeon (OMFS). (S)he will tell you what to do next. You may need a different antibiotic. Ask your dentist for a referral to an OFMS today.

See DDS: You didn't state whether the dentist performed any treatment other than an antibiotic. Is the infection related to specific teeth? If so, they need to be addressed or a referral to an oral surgeon may be advised.
