See a doctor now: Testicular pain although sometimes of no consequence, can be caused by infection or an interruption to the blood supply of the testis. Patients with testicular pain that doesn't settle quickly should therefore see a doctor right away

Infection?: In your age (assuming he's a similar age) infections are the common cause. A twisted testicle (torsion) is a more serious possibility (pain is usually sudden and severe). Other possibilities include trauma (accidental injury), kidney stones (gives referred pain), retrograde ejaculation (sperm goes the wrong way), vasocongestion (blue balls) from prolonged arousal without ejaculation.

See doctor urgently : Young man + unwell + painful testicle = testicle threatening nasties such as testicular torsion until proven otherwise. May also be the less but still serious infection or a hernia but needs examination. Most local emergency departments will give this issue a high priority. Please consult a doctor urgently

Dizzy & Pain: Your boyfriend needs to see a doctor right now. He MAY have an infection. IF the testicular pain is from a twist in his cord to the testicle it is a medical emergency. When he sees the doc he will know what is is dealing with. Peace and good health.
