Bacterial Vaginosis: If you have a new fishy odor with discharge the most likely diagnosis is Bacterial Vaginosis which can be treated empirically once full urine and blood sexually transmitted disease screening +preg test is performed. Vaginal swab also a great test but you need pelvic exam by OB/GYN to rule out other causes. ACOG lists new odor as an indication for a woman to seek OBGYN pelvic exam.

Bacterial vaginosis?: The main cause of strong vaginal odor, sometimes with discharge, is bacterial vaginosis, an overgrowth of mostly normal bacteria. There is debate about whether BV is considered an STD, but it's often triggered by sex, and other STDs (gonorrhea, chlamydia) are more common in women with BV.
