Resistance: You need to call your doctor and be seen again. You may be resistant to the antibiotic you are on and need a different one.

Return to doctor: Return to the doctor or clinic treating you for reevaluation and perhaps retreatment. The best way forward depends a lot on what you mean by "not working". If continued symptoms like vaginal discharge or abdominal pain, you may have had more than one problem -- gonorrhea cured but the other problem continuing. Resistance of gonorrhea to standard treatment is possible but very rare in US.
