Sesamoiditis: If the area under the big toe joint is painful, it is usually sesamoiditis or tendinitis or even a neuroma. Did you wear some high-heeled shoes and did alot of walking? Shock from the ground transferred into your big toe joint. It can be a result of wearing tight shoes also causing the toe tendon or the nerve to swell. Elevate the foot, wear wider tennis shoes with orthotics while the foot heals.

Shoe wear: A stiff soled, rocker bottom shoe with sufficent cushioning can decrease pain in the forefoot especially under the great toe.You should get evaluated by an orthopedic doc or or a podiatrist and get an x-ray you may have sesamiditis or a stress fracture at the very least.

I'd start with...: A "dancer's pad." i don't know why they are called this, but it is a pad worn on the bottom of the foot to accommodate the sesamoids under the first metatarsal head to keep them from bearing as much weight as they usually do. Then otc anti-inflammatory meds such as aleve (naproxen). If these don't help as much as you need, see a podiatrist.

Pain: All good answers and advice. Do see your podiatrist for an evaluation and x rays . As stated it could be a fracture also- the words extreme pain are the key here.
