What is your : Question. If the believe the information is not an accurate reflection of your encounter you could call the medical information department for correction of records. It could be possible that the supervising physician is attesting to the record

Attending supervised: Sometimes when a person goes to emergency room he or she is seen by a resident, third year or another physician covering for a physician. the next day that ER Board certified physician read the incident and added an addendum to what happened to clarify a miscommunication or to document information he or she obtained by talking to other staff who were present the previous day. more info helpfull.
