My boyfriend is having severe ear pain after using equate brand saline nasal rinse...should we go to the emergency room?

Hard to say: The ears and nose have no direct connection so I cant see where the nose spray has anything to do with it. The sudden ear pain could just be pressure (like landing in a plane) and yawning a few times could open the Eustachian tube and relieve it.It may also be a new onset ear infection, and an urgent care center would be a better place to start.

Severe ear pain: An urgent care center is often quicker and easier than going to an emergency room. Please do see a doctor. Peace and good health.
