Fissure: Sounds like you might have an anal fissure. See your doctor or ask to be referred to a surgeon for a proper exam. If it is a fissure, this can often be managed with an ointment that acts on the sphincter so it doesn't typically mean surgery, but colorectal surgeons are used to examining the area and making sure you are comfortable.
