It's like asking...: That is like asking, "How often do you misplace your car keys?" It happens, hopefully rarely, but likely varies from person to person (and varies from lab to lab, in the case of lab samples). All lab procedures have humans somewhere in the process... which means there is room for human error. Doctors usually know if one particular lab is overly prone to losing samples.
