Chronic sinusitis: Can cause such symptoms, short bursts of antibiotics often don't help, longer courses, 4-8 or even 12 weeks, at times are needed for eradication of infection, and possibly need for steroids as well. If an ENT evaluated you and offered surgery, then he / she has a good reason, at any rate, get a second or even third ENT opinion, if you're not sure, it's your right, good luck
