Multiple causes: strain, inflammation ,infection, epididymal cysts. You may need a testicle ultrasound and certainly a repeat exam.

Pain not normal.: We would not presume the experience of pain anyplace to be "normal". The cause for the pain may not be something dire or dangerous, but the presence of pain suggests that something's going on in the left testicle. Hydrocoele? Varicocoele? Epididymitis? This deserves an evaluation by a Urologist, and perhaps an ultrasound of the testicles.
