Hard to say: The human nervous system has no direct localizing fibers in the internal organs.You cannot itch inside your stomach.What type of sensation a 4yo would describe this way is hard to say. Gas/bloating/reflux/cramps ? Perhaps you can coax a better description if you work on it.
