Privacy : What reason do you want to keep it out of your medical records. It's important for your doctors to know about all medicines, vitamins, herbals that you are taking especially since some of these substance may interact with medications used in anesthesia. When you complete the privacy statement you can write on the form what information you don't want shared outside your procedure. But do tell him!

Better be honest: Your medical records are privileged and only you can agree to release them, unless there is some court order. You need to be honest and transparent and reveal to your doctors all your extraneous habits, as this is imperative for your safety.
