There will be blood: Yes, this is the title of a movie, but it is also the answer to your question if leeches have entered either area. There will be anal or vaginal bleeding noted in which case a MD visit is essential.

Examination: It is not very likely that leaches entered the vagina or anus unless they were placed there deliberately. A quick search of the medical literature documents only a few cases that have ever been found. In those cases the vaginal leaches were removed by a gynecologist.
