Plantar wart?: If it's a wart, they can freeze it with liquid nitrogen, not a needle. Or by "freeze" do you mean "numb"? If it is a plantar wart, it is safe to try over the counter treatment with Compound W or Duofilm for a few weeks before seeing a specialist. If it is a thorn or splinter, it probably needs removed sooner and they will numb it with a needle first.

Ah.. Come on....: It really depends on just what is deep in your foot as well as how deep is it. Foreign bodies and soft tissue masses located deep in the foot require some type of anesthesia. The choices are local, either with or without sedation or general. Superficial elements sometimes require anesthesia or, as in the case of warts, an assortment of non painful options are available. Just be sure to ask.
