For NEW syphilis...: The first sign of syphilis is a chancre (a ulcer or a sore that is painless). The sore appears at the spot where the bacteria got into the skin, and spreads into the body. Most people infected with syphilis only get one chancre. The chancre happens about three weeks after exposure. Many people who get syphilis don't notice the chancre because it's usually painless. Thus, people need blood testing.
