Wait and see...: I assumed you got scrotal US for local ache showing a 4-mm cyst-probably spermatocele, which is benign & harmless, and may slowly grow in size, but does not need care unless symptomatic. Of course, it is hard to give right answer without a detailed history + direct palpation.
