Possible dental prob: Without knowing more info, It sounds most concerning for possible dental caries with potential tooth abscess with refer pain of a headache/shoulder pain. Please see a dentist asap to get it checked out. Other less possible diagnosis will be TMJ problem, parotid gland issue, ear infection, lymphadenitis, etc.
