Consider PCR: Some cases of recurrent or chronic UTIs can be missed by using the routine culture technique. In your case, I believe a PCR assay for bacteria would be justified given that the current culture method is not that reliable. Looking for bacterial DNA with the PCR method may unveil the real culprit.
