If person gets same daily dose of methadone from a clinic, how/why can they still appear high and nod off, sleep, and be disoriented all the time?

Needs attention: Any patient on chronic narcotics who exhibits drowsiness or confusion, may either be on too high a dose or are taking additional narcotic medications, and in both cases this could be sign of future risk. Have attending doctor get aware of situation ASAP
