You Don't: If you have kidney failure it is painless. The kidney has nerves only on the outside of the kidney and they sense only stretch. Therefore, if you have an acute kidney infection with swelling of the kidney or a severe inflammatory kidney disease the kidney will hurt like a stone. Bilateral flank pain just above the rib cage, typically crampy in nature and not affected by movement or position.
