Prefer no surgery: Sounds like a fairly large disc protrusion, but if no progressive muscle weakness, atrophy or bladder involvement, might be able to get conservative PT, stretching, exercise, acupuncture, and try to allow disc to desiccate and shrink, which it may well do over 8-9 months. Key issue is presence or absence of neurological involvement. Steroids may help.

Varies: What you are describing is a disc herniation, which can heal over time or respond to nonoperative care including oral medications, traction, physical therapy, chiropractic care, or epidural steroid injections; among other options. Only a small percentage if patients require surgery, but because the condition is so common, it is one of the more common surgeries we do.
