Not necessarily: 2/3 out of the following are required to make the diagnosis of PCOS: 1) Irregular menstrual cycles. 2) Clinical or lab evidence of testosterone excess. 3) Exclusion of other disorders. In my clinical practice, I would first rule out hypothyroidism, Cushing's syndrome (if clinically indicated), hyperprolactinemia (if clinically indicated), non-classical congenital adrenal hyperplasia (if indicated)
