See a Provider!: Severe pain in the abdomen with radiation to the shoulder is worrisome. It may signify gallstones or irritation of the diaphragm from a perforation of an internal organ:colon, small bowel, fallopian tube or ovary, etc. It could also be something as simple as constipation and irritable bowel sx. So, see your provider and sort it out!
