See urologist or ID: Presumably tested for STDs? Gonorrhea and chlamydia are the most common causes of epididymitis or orchitis at your age. If diagnosis of non-STD bacterial infection is correct, treatment depends on which antibiotics are active against that bacteria. Usually Bactrim (sulfamethoxazole and trimethoprim) or a quinolone would work, but not always. Check for TB, testicular cancer. Discuss w/ urologist or an infectious diseases specialist.
