Yeast is unlikely: Symptoms like this almost never are due to yeast. It sounds like you have a prostate problem, or maybe a bacterial urinary tract infection. Conceivably an STD like gonorrhea, chlamydia, or nongonococcal urethritis (NGU), but these are relatively uncommon at your age. My bet is on prostatitis. See your doctor or urologist; or start with one of UK's excellent NHS GUM clinics.
