Surgically: Varicocoeles can be surgically treated by an urologist, however this is usually only done for those who are young and have difficulty conceiving due to poor sperm count. If one has children, and the varicocoele does not cause any problems, then it is usually left alone.

Varicocele: Unless your brother in law is having trouble conceiving children, having pain in that testicle, seeing that testicle shrink, or some other complaint, then i would leave it alone. Surgical clipping is common for them like hemorrhoids. Or if you have a good interventional radiologist you could embolize them without surgery, but still a procedure.
