Possibilities....: A steroid inhaler would get the steroids in high concentrations locally in the airways so it may help to add that. In addition, you need to be treated for GERD, especially given that your symptoms worsen when you lie down. Reflux can definitely give you the cough you describe. I would also treat for post-nasal drip by adding a steroid nasal spray. Hopefully, this will get rid of your cough!!!
