Does mono always cause a fever?

Depends: For you to recognize it as mono there is likely enough going on with the body that fever would be expected at some point. However, there are likely some that come in contact with the germ, become infected without any symptoms and show a positive blood test later."Sub-clinical" infections are a common way that germs spread in the community without people knowing where they got it.
